Information From CMS on Heart Failure Reimbursement

On July 18, 2014, CMS published a National Coverage Determination (NCD) with instructions regarding cardiac rehabilitation (CR) for heart failure (HF) patients.

The implementation date is August 18, 2014. This means that our Medicare contractors (known as MACs or Medicare Administrative Contractors) must have completed changes to their claims processing software so that appropriate heart failure diagnoses (ICD-9 codes) are not denied.

Medicare will now cover CR services to patients with stable, chronic heart failure defined as:

  • Patients with left ventricular ejection fraction of 35% or less, and
  • NYHA class II-IV symptoms despite being on optimal heart failure therapy for at least 6 weeks.

This is news that we have all been waiting for.

I will be in discussion with our MAC Medical Director for clarification of Diagnosis codes. However, most centers are using (ICD-9) code 428.22 (chronic systolic heart failure) with some success. Please work with your individual billing departments to ensure you are using the correct Heart Failure codes.

Stay tuned for further details and check back on the website for any updates. (the website will be updated by next week)

Best Regards,
